What do I need to provide on a VAP form?

From genetic testing to the Voluntary Acknowledgement of Paternity (VAP) form, there are a number of issues that people who wish to establish paternity in Illinois must understand. If you live in Belleville, or any other city in the state, and have decided to establish legal fatherhood by submitting a VAP form, it is crucial to know what you will need to provide when completing the form.

According to the Illinois Department of Public Health, you can fill out a VAP form if no father is on the child's birth certificate and his or her biological parents are unmarried. Parents may have to move forward with a Court Order of Paternity in some cases, such as either parent not being able to submit a Voluntary Acknowledgement of Paternity form.

When filling out a Voluntary Acknowledgement of Paternity form, you will need to provide the full name of the father as well as the state he was born in (or, if he was born outside of the U.S., his country of birth). The VAP form will also need to list the date that the father was born and include a copy of his government-issued picture identification, which cannot be expired. The child's mother must also provide a copy of her identification.

Before you submit a Voluntary Acknowledgement of Paternity form, it is essential to make sure that all of the required information is provided and that nothing is incorrect. Failing to properly submit a VAP form may cause the form to be delayed or even rejected.
